What is the Ulster County Health Home Program?

Through a partnership with the Ulster County Health Home, RCAL provides Care Management Services for qualifying adults or children receiving Medicaid. Care Managers provide service coordination, information and referrals to individuals in order to help consumers manage medical, mental health, housing, social service, education, vocational, transportation or any other identified need.

Adult Health Home Eligibility Requirements:

  • Medicaid recipient or Medicaid-qualified
  • 18 years or older
  • Having 2 or more chronic medical conditions OR 1 single qualifying condition
  • Diagnosed with HIV/AIDS or a serious mental illness

Children’s Health Home eligibility requirements:

  • Medicaid recipient or Medicaid-qualified
  • 0-21 years of age
  • Having 2 or more chronic medical conditions
  • Diagnosed with HIV/AIDS or a Serious Emotional Disturbance or documented Complex Trauma

The service is free. By joining the Health Home, you receive a Care Plan and support based on your needs. Your Care Manager will help you build on your team of providers, supports and services to help you improve your quality of life.
