THE PROGRAM
The program provides transition services to 9th grade students who have
been classified with a disability and who may be at risk of dropping out
of school. The program is designed to help these students become
more involved in their own transition planning.
WHAT IS
TRANSITION PLANNING?
 |
Transition
planning is a student-centered activity that requires a
collaborative effort. |
 |
Responsibilities are shared by the student, parents, and school
personnel, all of whom are members of the transition team.

 |
The
primary role of parents during transition planning is to
encourage and support students to plan and achieve
their educational goals. |
 |
Parents
also should encourage students to develop independent
decision-making and self-advocacy skills.

THE
STUDENTS
 |
Students
chosen to participate in the transition activities are those
with an Individual Education Plan (IEP) and who may be at-risk
of dropping out of school. |
 |
Students
participate in the activities in one or two groups based on
their need for transition guidance. |
 |
One group
consists of 5 to 10 students from each participating school.
These groups meet once a week for eight weeks.

The second
group consists of 10-15 students from each school. These
groups meet twice a month for the entire school year and explore
topics in greater detail. |
