Service Coordination

Service Coordination supports people with developmental disabilities in identifying and obtaining supports and services through choice and self-determination.  Service Coordination also provides oversight for any health issues.

RCAL's Medicaid Service Coordinators are experts on community services, resources and entitlement programs. They also act as brokers, advocating for access to programs and services under the direction of the individual consumer and are directly accountable to consumers and their families.

Although RCAL's Service Coordinators assist people in assessing services, they do not provide specific services themselves.

Some of the resources include:

  • Waiver Services
  • Housing
  • Personal assistance in ADL's
  • Employment
  • Medical/Clinical Services
  • Counseling
  • Respite
  • Special Equipment
  • Transportation
  • Recreation
  • Benefits and Entitlements
  • Environmental Modifications
  • Education
  • Other resources as requested, based on individual interests

RCAL provides Medicaid Service Coordination to people with developmental disabilities, including mental retardation, autism spectrum disorders, Cerebral Palsy, epilepsy and other neurological impairments.

To be eligible for Medicaid Service Coordination services, one must have a documented developmental disability occurring before the age of 22 and be Medicaid eligible.

Application for Medicaid Service Coordination can be initiated by calling RCAL at (845) 331-0541 or by calling the Taconic DDSO at (845) 382-1899 (ask for Intake).  You can also e-mail our Medicaid Service Coordinator Supervisor Chris Lewoc at clewoc@rcal.org if you have any questions.

We invite you to join us in "Opening the Doors of Opportunity."
